How can I affix my photo on the PAN Card?

I was minor at the time I received my PAN card. My photo was not included on my PAN Card because I was minor. Now, I am an adult. Please tell me how to get my photo onto a PAN Card. Do I need to fill out a corrections form?
Yes, you will need to complete the Change/Correction Request form. You can find it on the official website of NSL or UTI.
Please attach the following documents to your application: your ID Proof, Address proof, and DOB Proof along with your photo on Form.
As an adult, you can have your photo attached to the new PAN card.
